It is because of the gums disease which also called pyorrhoea. now the treatment and outcome is depends upon how advance is the gums problem. if in the beginning condition regular cleaning is enough. or need deep cleaning or flat surgery for advance tarter deposition. if the tooth is not savable than might need extraction.
